1. Acceptance of Terms
By accessing or using the Health Nourish Go website (vitallifehub.com) and any of our health & wellness services, you agree to be bound by these Terms & Conditions ("Terms"). If you do not agree, please discontinue use immediately. Your continued use constitutes acceptance of any updates we may publish.
2. Definitions
- "Company," "we," "us" refers to Health Nourish Go, a Utah-registered health & wellness provider.
- "Member," "you," "user" refers to any individual using our website or services.
- "Services" includes consultations, wellness programs, care coordination, telehealth, dental, dermatology, cardio, neuro, and insurance-support offerings.
- "Content" refers to all text, graphics, images, and information displayed on our website.
3. Services Description
Health Nourish Go provides concierge-style health & wellness management, including but not limited to: General Healthcare, Dental Care, Wellness/Nutrition/Weight Loss, Hair & Skin Treatment, Joint/Bone Health, Cardio Health, Men & Women Healthcare, Memory & Neuro Healthcare, and Health Insurance guidance. Specific services may vary by location, membership plan, and availability.
4. Membership & Billing
By subscribing to a paid membership, you authorize recurring charges to your designated payment method. Membership fees are billed monthly in advance. You may cancel anytime; cancellation takes effect at the end of the current billing cycle. Refunds for partial months are not provided. Price changes will be communicated at least 30 days in advance.
5. Appointments & Cancellations
Appointments must be scheduled in advance through our website, phone, or your coordinator. We require at least 24 hours' notice for cancellations or rescheduling. Repeated no-shows may result in a fee and possible suspension of same-day booking privileges.
6. No Medical Advice Disclaimer
Information on our website is for general educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified health provider with any questions regarding a medical condition. Never disregard professional medical advice due to content on our site.
7. Limitation of Liability
To the fullest extent permitted by law, Health Nourish Go, its directors, employees, partners, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages — including loss of profits, data, or health outcomes — arising from your use of our services. Our total liability for any claim shall not exceed the amount you paid us in the prior 12 months.
8. Intellectual Property
All website content, branding, logos, graphics, and code are the exclusive property of Health Nourish Go and protected by U.S. intellectual property laws. You may not reproduce, distribute, or create derivative works without our prior written consent.
9. User Conduct
You agree to: (a) provide accurate, current information; (b) not misuse our services, including harassment of staff or other members; (c) not attempt to disrupt, hack, or reverse-engineer our systems; (d) comply with all applicable laws, including HIPAA.
10. User-Generated Content
If you submit reviews, testimonials, or feedback, you grant us a non-exclusive, royalty-free, worldwide license to use, display, and adapt that content for marketing and improvement purposes. You warrant that you own the rights to content you submit.
11. Third-Party Services
We partner with external clinics, diagnostic labs, payment processors, and technology providers. We are not responsible for the practices or outcomes of third-party services. Your engagement with them is governed by their respective terms.
12. Telehealth Consent
Telehealth services use secure video, phone, and messaging technology. You acknowledge that telehealth may have technical limitations and is not appropriate for emergencies. In a medical emergency, call 911 immediately.
13. Privacy & Data
Your use of our services is also governed by our Privacy Policy, which is incorporated into these Terms by reference.
14. Refund Policy
Membership fees are non-refundable once a billing cycle has begun. One-time services (e.g., consultations) may be refunded if canceled with at least 48 hours' notice and no clinician time has been incurred. Refund requests can be sent to info@healthnourishgo.com.
15. Newsletter & Marketing
By subscribing to our newsletter, you consent to receive marketing emails. You may unsubscribe at any time via the link in each email or through our unsubscribe page.
16. Governing Law
These Terms are governed by the laws of the State of Utah, USA, without regard to conflict-of-law principles. Any disputes shall be resolved in the courts located in South Jordan, Utah, unless otherwise required by mandatory consumer-protection laws.
17. Dispute Resolution
We are committed to resolving disputes amicably. Any dispute will first be addressed through good-faith negotiation. If unresolved within 60 days, the matter will proceed to binding arbitration in Utah under AAA rules. Class actions are waived to the maximum extent permitted by law.
18. Changes to Terms
We reserve the right to update these Terms at any time. Material changes will be posted on our website and, where relevant, communicated via email. Continued use after changes constitutes acceptance.
19. Severability
If any provision of these Terms is found unenforceable, the remaining provisions shall remain in full force and effect.
